Community health centers by state
Federally qualified health centers (FQHCs) are community-based clinics that provide primary care to anyone, regardless of ability to pay. Each year every health center files a Uniform Data System (UDS) report with HRSA covering its patients, visits, staffing, clinical quality measures, costs and revenue. This directory turns 2025's reports into one page per state and per health center, with 2016–2025 trends and state and national benchmarks.
